Human resources - please see HR tab

  • New hire or rehire paperwork questions?  Email payroll@chicago-scoops.com
    • Instructions for Paychex on-boarding and troubleshooting can be found on the HR tab
      • If not working & need to send hard copies of paperwork: utilize new hire packet on HR tab, plus the appropriate state W4 where applicable.
    • Acceptable I9 documents/IDs list on HR tab.
    • Rehires should not be entered into Paychex onboarding but fill out the packet on the portal.
    • Address, name, or withholding change?  Email a new W4 (and I9 if name change) to payroll@chicago-scoops.com.
    • NYC premiums for shift changes, etc. should be emailed to payroll@chicago-scoops.com.  This includes Clopen forms.
    • Store transfer forms should be sent to payroll@chicago-scoops.com, especially for permanent changes from 20619 to another store.  If temporary, please inform payroll.
  • Paycheck or W2 questions?  Email payroll@chicago-scoops.com
    • If it's simply wanting access to a paystub or W2, refer to instructions on HR tab.  This can be shared with current and former employees inquiring.
    • If there's a mistake on the W2, please email payroll.
  • Direct deposit question or submission?  Send to payroll@chicago-scoops.com
    • Form available under HR tab.
    • New hires who sign up directly through the Paychex onboarding system will need to send over supporting documents such as a voided check or bank letter for the enrollment to be confirmed.  Until this is received, they will be issued live/paper checks.
    • Rehires must be sent separately from on-boarding
    • Must send Paychex direct deposit form along with voided check or bank letter for activation to take place.
    • Changing direct deposit may require a Stop Direct Deposit form and new direct deposit form with supporting voided check or bank letter.
  • Lost or stolen check?  Email payroll@chicago-scoops.com
    • Fill out lost check affidavit by utilizing the paystub for employee (see how to access above) & where to send the reissued check (meaning address of store or employee's home).
  • Incident report submission or question?  Email payroll@chicago-scoops.com.
    • ​Form available on HR tab.  Print off and fill out accordingly, sending along pictures if helpful/necessary.  HR will submit to insurance.  Any additional expenses or bills should also be sent to HR so they can be submitted to our insurance for reimbursement/payment.

  • Missed punches?
    • Make sure to change date when correcting & email payroll what change was made
  • Separation report, termination, or writeup questions?  Email payroll@chicago-scoops.com
    • Print separation or writeup form, complete, and send pic or scan to payroll@chicago-scoops.com
  • Wage change questions or submission?  Email payroll@chicago-scoops.com
    • Print wage change, complete, and send pic or scan to payroll@chicago-scoops.com, cc'ing your supervisor for approval.
    • Subject line for email should be Store number - Employee Name - Document Type.
      • Pay period calendar available on HR tab for effective date reference.
  • PTO (Paid Time Off) questions or submission?  Email payroll@chicago-scoops.com
    • Instructions and policy can be found on the HR tab
    • What's the difference between sick/personal time & vacation time?
      • Sick/personal time should be used for last-minute and usually unplanned absences.  Vacation time should be used for planned absence.  There are two separate "banks" of hours that you accrue per pay period depending on how long you've been with the company.

  • Where is the corporate office and when are they open?
    • Chicago Scoops, LLC: 1901 Clybourn Avenue, Suite 401, Chicago, IL 60614.
    • Hours are 9am to 6pm from Monday through Friday.
    • In case of emergency after hours or on weekends, contact your supervisor.

  • Electricity, utility, or internet issues?  Contact the corporate office immediately - 312.755.6723 (Dayna - Office Administrator).
  • Email or portal login issues?  Contact info@chicago-scoops.com.
  • Theft suspicion (internal employee or customer)?  Contact the corporate office immediately - 312.767.2050 (Andrew - Controller) as well as your supervisor.
  • Did you receive important-looking mail at the store?
    • Anything employment-related (past or present) or pertaining to an employee should be sent to alybitter@chicago-scoops.com.  This also includes child support orders.
    • Bills or invoices to accountspayable@chicago-scoops.com.
  • Need to order something?  Contact your supervisor directly.
    • Business card orders can be emailed to alybitter@chicago-scoops.  Please keep in mind that, as a general rule of thumb, we usually order store-related business cards sans a specific name for GMs.
    • Iron Mountain supplies & pickups can also be ordered through your supervisor.
  • Need to have something repaired?  Contact your supervisor directly.
  • What's the difference between Accounts Payable and Payroll?
    • Accounts Payable has to do with billing and payments to vendors for services rendered.  Payroll has to do with paying employees.
